Form-Ⅰ (General Information) should be completed by the oil company witness and logging company engineer prior to the logging operation. Form-Ⅱ（Planned Logging Program）should be completed by the oil company witness prior to the logging operation During and after logging Form-Ⅲ (Quality Control Check List) should be completed by the logging company engineer.
