[Abstract] Objective To evaluate the cost-effectiveness of Oseltamivir in China along with a randomized, control and open clinical trial. Methods A randomized, controlled clinical trial was conducted. The sample size included 118 influenza-like cases, among them 56 cases were confirmed by virus isolation and serological test. Cost data was collected from direct medical cost （ambulatory visit and hospitalization）and the cost for adverse events. The indirect cost was calculated by human capital method. The outcome was measured by the number of rest day, the average day of work loss, average time to clearance of symptoms and fever, and percentage of improvement of symptom scores compared to the control group. Results it shows that the medical direct cost 145-200 yuan (RMB) could be saved in each case treated by Oseltamivir. It is related to the diminution of hospitalization, complications and less use of antibiotics. Oseltamivir has good efficacy, the average duration of shorten fever was 2.35 days，the symptoms released about 4 days. It will totally shorten 2-2.5 days.
